Centre Original Forms
If you would like to continue to collect the data that has not been included on the forms you are able to do so. This will require you to remake some of your centre forms so that parents are not duplicating the same questions.
It is important to note that questions from the standard forms should not be duplicated on any centre forms other than Name, Surname, DOB and Postcode. This will help to ensure that centres use the most up to date question sets. For example, we want to ensure that centres do not collect ethnicity information from an incorrect or out of date question set.
